Each participant for this event MUST be a current member of the Ultimate Players Association.  Be sure that all members of your team are current on their UPA dues and that they've signed the UPA waiver and release form.

If needed, you and your teammates can join the UPA online.  (You can use the same link to check membership status)

Rosters are due to the UPA by August 29, 2003 
Please complete your roster and mail it to the UPA so that it will be delivered by August 29.  Getting the rosters in early makes it so much easier on our friends at Headquarters.  There will be a $50.00 late fee for any roster received after August 29.

This is what you'll need:
1) Completed UPA Team Roster
2) Completed UPA Waivers 
3) UPA membership dues checks made out to  "UPA"  

Pack it all up and send it to the Ultimate Players Association
